Types of Diabetes We Manage

Diabetes is not a single disease. Each form requires a different clinical approach, and identifying the correct subtype helps guide appropriate long-term management. Our consultants manage the following:

Type 1 Diabetes

Type 1 diabetes is an autoimmune condition in which the immune system damages the insulin-producing beta cells of the pancreas. People with Type 1 diabetes need insulin treatment for life. Our service supports multiple daily injection regimens, insulin pump therapy, continuous glucose monitoring (CGM), and structured carbohydrate counting.

Type 2 Diabetes

Type 2 diabetes results from a combination of insulin resistance and progressive failure of insulin secretion. We provide individualised care plans that integrate medical nutrition therapy, physical activity, oral therapies including modern agents such as SGLT2 inhibitors and GLP-1 receptor agonists, and where clinically appropriate, insulin therapy. For some patients with early Type 2 diabetes, structured remission pathways may be discussed. Whether this is realistic depends on individual clinical assessment.

Gestational Diabetes

Gestational diabetes is raised blood glucose that is first recognised during pregnancy and that does not meet the criteria for diabetes outside pregnancy. Where blood glucose is high enough to suggest previously undiagnosed diabetes, this is assessed and managed differently, which is one reason accurate diagnosis matters.

Prediabetes

Prediabetes is a state of elevated blood glucose that does not yet meet diagnostic criteria for diabetes but signals materially raised future risk. Early specialist input can support sustainable lifestyle change, address insulin resistance, and where appropriate, consider medical therapy that aims to delay progression.

MODY and Other Monogenic Diabetes

Maturity-Onset Diabetes of the Young (MODY) is caused by single-gene mutations and can be mistaken for Type 1 or Type 2 diabetes. Correct genetic diagnosis affects treatment choice. We coordinate genetic testing where the clinical picture suggests a monogenic cause.

Type 3c (Pancreatogenic) Diabetes

Type 3c diabetes develops following pancreatic disease such as chronic pancreatitis, pancreatic surgery, cystic fibrosis or pancreatic cancer. It carries unique management considerations including pancreatic enzyme replacement, vitamin status review, and individualised insulin titration.

Diabetes Diagnostic and Monitoring Services

Clinical decisions are guided by accurate diagnostic data. The clinic offers the following diagnostic and monitoring services, with most blood and urine investigations performed through our partner accredited laboratories.

  • HbA1c testing

    A laboratory measurement that reflects average blood glucose over the previous two to three months, used both for diagnosis and ongoing monitoring.

  • Oral Glucose Tolerance Test (OGTT)

    Used in the assessment of prediabetes, gestational diabetes screening, and unclear cases.

  • Fasting glucose and fasting insulin

    Used to evaluate insulin resistance and metabolic dysfunction; helpful in patients with polycystic ovary syndrome, fatty liver or central adiposity.

  • Continuous Glucose Monitoring (CGM)

    A small sensor that measures glucose in the fluid just under the skin every few minutes, allowing us to see glucose patterns, overnight lows, post-meal rises and time in range. Whether CGM is appropriate for you, and which sensor to use, is decided during consultation.

  • C-peptide and autoantibody testing

    Used to clarify diabetes subtype, particularly in adult-onset cases where Type 1, Type 2 or MODY remain unclear.

  • Diabetes complications screening

    Renal function (eGFR, albumin-to-creatinine ratio), lipid profile, blood pressure, foot examination, and referral coordination for diabetic retinopathy screening.

Clinical note: Investigations are ordered based on individual clinical need following consultation. Not every patient requires every test. We will explain why each test is being recommended before it is performed.

Evidence-Based Treatment Approaches

Our treatment approach is guided by the MOH Clinical Practice Guidelines on Diabetes Mellitus, the ACE Clinical Guidances that update them, and current international consensus statements. Treatment is individualised, reviewed regularly, and decided together with you.

Lifestyle and Medical Nutrition Therapy

Structured guidance on carbohydrate quality and quantity, portion control, meal timing, and physical activity. Our doctors work alongside dietitian partners and, where relevant, exercise physiologists. The LiDDLe programme, adapted by Dr Ben Ng for Asian eating patterns, is available for suitable patients.

Medication (non-insulin therapies)

Diabetes medication now extends well beyond metformin. Depending on the clinical picture, treatment may include oral medicines and injectable non-insulin medicines such as GLP-1 receptor agonists, as well as SGLT2 inhibitors. In clinical trials, certain medicines in these classes have been shown to reduce cardiovascular and kidney complications in particular groups of patients, and this may influence which medicine is chosen for you. Treatment selection also takes account of kidney function, heart history, weight, the risk of low blood sugar, cost and your own preferences.

Insulin Therapy

Insulin therapy may be initiated when oral or injectable non-insulin agents no longer maintain glycaemic targets, in Type 1 diabetes from diagnosis, in selected cases of gestational diabetes, and in some patients with newly diagnosed Type 2 diabetes with very high HbA1c. We provide education on dose titration, injection technique, hypoglycaemia recognition and management, and sick-day rules.

Insulin Pump Therapy and CGM

Insulin pump therapy delivers insulin continuously through a small device rather than through separate injections, and can be paired with continuous glucose monitoring so that insulin delivery responds to glucose readings. Some patients, particularly those with Type 1 diabetes, find this allows finer adjustment of insulin. Pump therapy involves ongoing self-management and is not appropriate for everyone. Suitability is assessed individually.

When to See a Diabetes Specialist Instead of a GP

Most uncomplicated Type 2 diabetes can be managed appropriately at the GP or polyclinic level. A specialist review may be considered in the following clinical situations:

  • Your HbA1c remains above your agreed target after standard oral medications have been tried at appropriate doses.
  • You experience frequent or severe hypoglycaemia, especially overnight or unrecognised episodes.
  • You are planning a pregnancy, are pregnant, or have been told you have gestational diabetes.
  • You have been diagnosed with Type 1 diabetes, or your diabetes subtype is unclear.
  • You are starting or already on insulin therapy and would like to optimise your regimen.
  • You are interested in insulin pump therapy or continuous glucose monitoring.
  • You have developed a complication of diabetes affecting your kidneys, eyes, nerves, feet or cardiovascular system.
  • You have prediabetes alongside other features of insulin resistance such as fatty liver, polycystic ovary syndrome, or significant central weight gain.
  • You have diabetes following pancreatic surgery, chronic pancreatitis, or another pancreatic condition (Type 3c).

In Singapore you do not require a GP referral to consult an endocrinologist privately. You may contact the clinic directly. If you have insurance, your insurer may have specific referral requirements, which our staff can assist you in clarifying.

The role involves confirming the diabetes subtype, recommending a treatment plan, prescribing and adjusting medications including insulin, interpreting continuous glucose monitoring data, screening for complications, and coordinating care with related specialists such as cardiologists and nephrologists.

When should I see a diabetes specialist instead of a GP?

A specialist review is reasonable when blood glucose remains above target despite standard medication, when you experience frequent or severe hypoglycaemia, when you are planning a pregnancy or are already pregnant, when you have Type 1 diabetes or an unclear diabetes type, when you wish to consider insulin pump therapy or CGM, or when complications affecting the kidneys, eyes, nerves or heart have developed.

Do I need a GP referral to see a diabetes specialist in Singapore?

No. In Singapore you may book a privatespecialist consultation directly without a GP referral letter. That said, insurance plans sometimes require a referral for cover; check with your insurer. If you do have a recent referral letter or recent test results from your GP, please bring them along as they help inform our initial assessment.

What tests does a diabetes specialist do at the first consultation?

The first consultation usually begins with a structured history and physical examination. Tests commonly arranged include HbA1c, fasting glucose, lipid profile, kidney function, urine albumin-to-creatinine ratio, and thyroid function. In selected cases we may add C-peptide, diabetes autoantibodies, fasting insulin, or arrange continuous glucose monitoring. The exact panel is decided on individual clinical needs.

How much does a diabetes specialist consultation cost in Singapore?

Our indicative fee for a first specialist consultation is $180 to $320, and $120 to $220 for a follow-up. Investigations are charged separately and an itemised quotation is available on request. Can a diabetes specialist help with insulin resistance and prediabetes?

Yes. Prediabetes and insulin resistance are worth reviewing early, because this is the stage at which progression to Type 2 diabetes may still be potentially delayed or managed. A consultation will look at fasting insulin, body composition, fatty liver risk, and lifestyle factors, and discuss evidence-based options ranging from structured nutrition and exercise interventions to medication where appropriate.

What is continuous glucose monitoring (CGM) and is it suitable for me?

CGM is a small sensor worn on the upper arm or abdomen that measures glucose in the interstitial fluid every few minutes and transmits readings to a phone or reader. It is particularly useful for Type 1 diabetes, patients on insulin with frequent hypoglycaemia, gestational diabetes, and any patient wanting to understand how meals and activity affect their glucose. Suitability and sensor choice will be discussed during your consultation.

Can Type 2 diabetes be reversed or put into remission?

Remission is generally defined as an HbA1c below the diabetes range, measured at least three months after stopping glucose-lowering medication.Remission is not the same as a cure; ongoing follow-up remains important because the underlying tendency persists. Whether remission is a realistic goal for an individual patient is best discussed in consultation.

I am pregnant and have just been told I have gestational diabetes. Can you help?

Yes. Gestational diabetes requires prompt review because it carries implications for both maternal and foetal health. We can usually arrange an early appointment, coordinate with your obstetrician, set up home glucose monitoring, advise on dietary adjustments, and where required, initiate insulin regimens that are generally considered safe during pregnancy.

What should I bring to my first diabetes consultation?

Please bring a list of your current medications (or the boxes themselves), recent blood test results within the past twelve months, any glucometer or CGM device or app data, your GP referral letter if you have one, your identification, and your insurance details if relevant. Writing down your symptoms and questions in advance helps make the most of the consultation time.
