What Is Hyperthyroidism?

The thyroid is a small butterfly-shaped gland at the front of the neck. It produces thyroid hormones that help regulate energy use, body temperature, heart rate, digestion and many other body functions.

Hyperthyroidism, also called an overactive thyroid, occurs when the thyroid produces more thyroid hormone than the body needs. This is different from hypothyroidism, where the thyroid does not produce enough thyroid hormone.

Hyperthyroidism is not one single disease. It is a thyroid hormone state that can be caused by different conditions, including Graves’ disease, toxic thyroid nodules, thyroiditis, iodine-related causes or certain medications. Identifying the cause helps guide treatment.

Is My Thyroid Overactive? Symptoms to Watch For

Overactive thyroid symptoms can come on gradually or feel sudden. Some people notice obvious physical changes, while others feel generally unwell, anxious or unusually tired despite sleeping enough.

Hyperthyroidism symptoms can include fast heartbeat, tremors, weight loss, heat intolerance, sweating, anxiety, frequent bowel movements, tiredness, muscle weakness, sleep problems, menstrual changes, neck swelling and eye changes. Symptoms should be assessed with thyroid blood tests and medical review.

  • Fast heartbeat or palpitations: Your heart may feel like it is racing, pounding or beating irregularly.
  • Unexplained weight loss: Weight may drop even when your appetite is normal or increased.
  • Heat intolerance and sweating: You may feel warmer than others in the same room or sweat more easily.
  • Trembling hands: A fine tremor may be noticeable when holding objects or stretching out your hands.
  • Anxiety, nervousness or irritability: Symptoms can feel like stress, but may be driven by thyroid hormone excess.
  • Frequent bowel movements or diarrhoea: Digestion may speed up.
  • Tiredness or muscle weakness: You may feel weak in the thighs, arms or shoulders, even with daily tasks.
  • Sleep disturbance: You may struggle to fall asleep or wake frequently.
  • Menstrual changes: Periods may become lighter, irregular or less frequent.
  • Neck swelling or eye changes: Some patients notice a swelling in the neck, prominent eyes, eye irritation, blurred vision or double vision, especially with Graves’ disease.

When Should You See an Endocrinologist?

Consider seeing an endocrinologist if you have symptoms of an overactive thyroid, abnormal thyroid function test results, recurrent thyroid hormone imbalance, thyroid nodules, Graves’ disease, pregnancy-related thyroid concerns or uncertainty about treatment options.

You should also arrange a medical review if symptoms are affecting sleep, work, exercise, school, fertility planning or daily confidence. Palpitations, tremors and unexplained weight loss can feel frightening, especially when the cause is unclear. A structured assessment can help identify whether the thyroid is involved and what should happen next.

When Symptoms May Be Urgent: Seek urgent medical attention if thyroid symptoms are severe or come with chest pain, fainting, severe breathlessness, confusion, high fever, dehydration, marked weakness or a very fast irregular heartbeat.

Hyperthyroidism vs Hypothyroidism

Many patients search for the difference between hyperthyroidism and hypothyroidism because the names sound similar. They are opposite thyroid hormone problems, although some symptoms can overlap.

Feature Hyperthyroidism Hypothyroidism
Plain-English meaning Overactive thyroid; too much thyroid hormone Underactive thyroid; too little thyroid hormone
Common body effect Body functions may speed up Body functions may slow down
Possible symptoms Fast heartbeat, tremors, weight loss, heat intolerance, sweating, anxiety, frequent bowel movements, menstrual changes Tiredness, weight gain, cold intolerance, constipation, dry skin, low mood, heavy or irregular periods
Typical blood test pattern Low TSH with high free T4 and/or free T3, depending on the stage and cause High TSH with low free T4 in many cases of primary hypothyroidism
Common treatment direction Reduce excessive thyroid hormone production or its effects Replace insufficient thyroid hormone when clinically indicated
Why diagnosis matters Untreated hyperthyroidism may affect the heart, bones and general health Untreated hypothyroidism may affect energy, cholesterol, fertility, pregnancy and general health

Causes and Risk Factors

Several factors can lead to the overproduction of thyroid hormones, with some individuals having a higher predisposition to developing this condition.

  • Autoimmune Disorders

Graves’ disease, where antibodies stimulate the thyroid to produce excess hormones, accounts for approximately 80% of hyperthyroidism cases.

  • Thyroid Nodules

One or more lumps can develop in the thyroid gland, which may become overactive and produce excess hormones without responding to the body’s usual regulatory signals.

  • Excessive Iodine Intake

Consuming large amounts of iodine through diet or medications can trigger overproduction of thyroid hormones in susceptible individuals. This condition is rare in Singapore due to its close proximity to the sea.

  • Thyroiditis

Inflammation of the thyroid gland can cause stored hormones to leak into the bloodstream, resulting in temporary hyperthyroidism.

  • Gender and Age

Women are 5-10 times more likely to develop hyperthyroidism, with the condition most commonly occurring between ages 20-40.

  • Family History

Individuals with relatives who have thyroid disorders face an increased likelihood of developing hyperthyroidism.

Types of Hyperthyroidism

The classification of hyperthyroidism depends on the underlying cause, with each type presenting distinct characteristics that influence treatment approaches.

Graves’ Disease

This is an autoimmune condition where antibodies stimulate the thyroid to produce excessive hormones. This type often runs in families and frequently affects multiple body systems, including the eyes and skin. The thyroid gland typically enlarges uniformly, and specific antibodies can be detected in blood tests.

Toxic Nodular Goiter

This type develops when small, round growths called nodules form in the thyroid tissue. These nodules function independently from normal thyroid regulation, producing excess hormones regardless of the body’s needs. The condition typically progresses slowly and occurs more frequently in older adults.

Subacute Thyroiditis

This is a temporary form of hyperthyroidism that occurs when the thyroid gland becomes inflamed, often following a viral infection. The inflammation causes stored thyroid hormones to leak into the bloodstream. This type usually resolves within a few months, though it may be followed by a period of hypothyroidism before returning to normal function.

How Hyperthyroidism Is Diagnosed

  • Blood Tests: Blood tests are the primary method for assessing suspected hyperthyroidism. They measure T3 (triiodothyronine) and T4 (thyroxine) levels, which are usually elevated, while TSH (thyroid-stimulating hormone) is typically very low. These tests also help detect thyroid antibodies, which may help confirm autoimmune conditions like Graves’ disease.
  • Radioactive Iodine Uptake Test: This imaging test helps identify the cause of an overactive thyroid. Patients take a small dose of radioactive iodine, and specialised cameras measure how much iodine the thyroid absorbs. The absorption pattern can help distinguish conditions like Graves’ disease from thyroiditis or toxic nodular goitres.
  • Ultrasound: This non-invasive test uses sound waves to produce detailed images of the thyroid. It helps detect nodules, measures their size, and helps determine whether they are solid or fluid-filled, providing valuable information for treatment planning.

Treating Hyperthyroidism in Singapore

Hyperthyroidism treatment depends on the cause, severity, age, pregnancy status, symptoms, other health conditions, thyroid size, eye involvement and patient preference. Not every patient needs the same treatment pathway.

At Arden Endocrinology Specialist Clinic, treatment planning may involve discussing the benefits, limitations, risks and monitoring requirements of each option. The goal is to reduce excess thyroid hormone activity, manage symptoms and monitor for changes over time.

Antithyroid Medications

Antithyroid medicines work by reducing thyroid hormone production. Medicines used for hyperthyroidism can include carbimazole, methimazole or propylthiouracil, depending on clinical context. Your endocrinologist will advise on suitability, dose and follow-up blood tests.

Monitoring is important. Dose adjustments may be made according to thyroid blood test results and symptoms. Patients should be told what side effects to watch for, including fever, sore throat, mouth ulcers, rash, yellowing of the eyes or skin, or unusual tiredness.

Beta Blockers for Symptom Control

Beta blockers may be used for short-term control of symptoms such as tremors, palpitations and fast heart rate. They do not correct the underlying thyroid hormone excess, so they are usually considered alongside treatment of the underlying cause.

Radioactive Iodine Treatment

Radioactive iodine treatment may be discussed for some patients with hyperthyroidism. It works by targeting thyroid cells that take up iodine, reducing thyroid hormone production over time. It is not suitable for everyone, including pregnant women, and may lead to hypothyroidism that needs long-term thyroid hormone replacement.

Surgery Referral Where Appropriate

Surgery may be considered in selected situations, such as a large goitre, compressive neck symptoms, suspected cancer, specific pregnancy planning considerations, medication intolerance, or when other treatments are unsuitable. If surgery is needed, your endocrinologist can discuss referral and pre-surgery thyroid stabilisation.

Specialist Monitoring and Follow-Up

Follow-up blood tests help assess whether thyroid hormone levels are improving, whether medication doses need adjustment and whether treatment has led to hypothyroidism. Monitoring also helps identify relapse, treatment side effects and heart-related concerns.

Have Palpitations, Tremors or Abnormal Thyroid Blood Test Results?

A consultation with Arden Endocrinology can help clarify whether your symptoms are related to hyperthyroidism and what treatment options may be suitable for your condition.

Hyperthyroidism, Fertility and Pregnancy

Hyperthyroidism can affect menstrual regularity, fertility planning and pregnancy care. If you are pregnant, planning pregnancy or breastfeeding, tell your endocrinologist early because medication choices and monitoring needs may differ.

Patients should not stop prescribed thyroid medication without medical advice. Poorly controlled thyroid hormone levels can create risks, while overtreatment can also be harmful. This section should be reviewed by the named medical reviewer before publication.

Living With Hyperthyroidism During Treatment

While treatment is being adjusted, patients may still experience fatigue, changes in exercise tolerance, sleep difficulty, emotional changes or palpitations. It can help to track symptoms, resting heart rate, medication doses and blood test dates.

Avoid starting iodine supplements, thyroid support supplements or herbal products without checking with your healthcare professional. These may interfere with thyroid control or blood test interpretation.

Prevention and Management

Effective management of hyperthyroidism requires regular monitoring of thyroid function through blood tests and timely adjustments to treatment plans. Maintaining a consistent medication schedule and attending follow-up appointments with healthcare providers are necessary for optimal control.

A balanced diet supports overall well-being, though certain dietary restrictions may be needed depending on the treatment approach. Moderate physical activity tailored to symptom severity can improve general health, while stress management techniques may help reduce the risk of symptom flare-ups.

Frequently Asked Questions

What are the early signs of an overactive thyroid?

Early signs can include palpitations, feeling unusually warm, hand tremors, unexplained weight loss, nervousness, increased sweating, frequent bowel movements and difficulty sleeping. Some patients also notice neck swelling or eye changes.

What happens if hyperthyroidism remains untreated?

Untreated hyperthyroidism can lead to heart problems, bone density loss and a severe condition called thyroid storm, which requires immediate medical attention. Regular monitoring and adherence to prescribed treatment can help reduce these risks.

What is radioactive iodine treatment?

Radioactive iodine treatment targets thyroid cells that take up iodine and may reduce thyroid hormone production over time. It is not suitable for everyone, including pregnant women, and requires counselling on precautions and follow-up.

Will I become hypothyroid after hyperthyroidism treatment?

Some treatments, especially radioactive iodine or surgery, may lead to hypothyroidism. If this happens, thyroid hormone replacement may be needed and monitored with blood tests.

Can hyperthyroidism affect pregnancy?

Yes. Thyroid hormone levels need careful management before and during pregnancy. Tell your doctor if you are pregnant, planning pregnancy or breastfeeding because treatment options and monitoring may change.

Should I see an endocrinologist if my thyroid blood test is abnormal?

An endocrinologist can help interpret abnormal thyroid blood tests, assess symptoms, identify the cause and discuss treatment or monitoring options. This is especially helpful if results are persistent, severe, recurrent or linked to pregnancy, nodules or heart symptoms.
